Provide the following information via email to IDDReferrals@metrocareservices.org

a. Full name of parent/guardian and individual being referred
b. Contact number of individual/family
c. Copy of medical records stating diagnosis(es)
d. Copy of all psychological or psychiatric evaluation
e. Copy of the most recent Full and Individual Evaluation (FIE)

For Community Living Assistance and Support Services (CLASS), and Medically Dependent Children's Program (MDCP), Deaf Blind Multiple Disabilities (DBMD).

b. Date of Birth
c. Address
d. Phone number
e. Short questionnaire 10-15 minutes in length
f. Form will be sent in the mail annually to complete or call to maintain status on the list.

For Youth Empowerment Services (YES)

a. Leave a message with your name, child's name and phone number.  Your call will be returned in 24 business hours.
b. An intake will be offered within 7 business days. 
c. The intake will take between 1 1/2 - 2 hours with both child and guardian.
d. Intake location: 8915 Harry Hines Blvd. Dallas, TX 75235.
e. If your child is currently receiving Medicaid, the process will take between 2-3 weeks. If your child is not currently receiving Medicaid, then the process is dependent upon Medicaid.
