Medication may be administered by designated Coppell ISD personnel in compliance with district procedures. All prescription and non-prescription medication dispensed in the clinic must have a signed physician/health care provider order. While CISD has required a signed order from a medical provider for over-the-counter medication previously, now all medication must have a signed medical provider’s order. If you have any questions, please contact your child’s school nurse. There will be a grace period for this change regarding prescription medications through the first semester of the 2024-2025 school year.
This form can be used by the medical provider for both prescription and non-prescription medications.
